Media Consent
BASE Camp photos, videos, and audio recordings help support our educational mission. Most photos and videos are of groups (since much of BASE Camp is a team event). You can see examples on our website.
Only “flattering” media are selected! (the ones that show people doing the best things!)
And, if you’d like we’d be happy to share any photos with you for permission before posting.
I consent to be in photos, videos, and audio clips taken during BASE Camp educational activities at Weill Cornell Medicine and NewYork Presbyterian Hospital, and agree to the following:
- I give my consent and authorize BASE Camp, Weill Cornell Medicine (WCM) personnel, and NewYork-Presbyterian Hospital staff to photograph, video record and/or audio record me during simulation activities.
- I understand and agree that such photographs, video recordings, and/or audio recordings may be used for scientific, educational, and/or public relations purposes – such as on our website, brochures, flyers, newsletters, presentations, and/or periodicals.
- I have been advised and understand that:
• I have the right to request that such photographing or recording be stopped; and,
• I have the right to rescind my consent for use up until a reasonable time before the recording or photo is used.
• I have the right to refuse consent to photographing or recording and understand that this decision will not affect my right to participate in this course. - Finally, I release BASE Camp, WCM personnel, and NewYork-Presbyterian Hospital staff from any claim, liability, or responsibility – including payment of royalties or other compensation – that I may have by reason of photographing, video recording, and/or audio recording or the uses mentioned in Part 2.
